Arrange for Consultation

To learn more about CMHI, the medical home, and to gain support with primary care transformation you can access CMHI’s consultation resources through a variety of means, at scalable cost levels.

 

Check with our office (Lori.Keehl-Markowitz, 603-228-8111) for more information.  CMHI can:

  1. Use the TAPPP™ Analysis process including an assessment, report, recommendations, tools, and curriculum in a variety of productive ways.
  2. Offer a variety of tools as templates or individualize them to meet your needs
  3. Contract with organizations or individuals to help with the accomplishment of goals. For example, CMHI will work with its clients to establish a work plan, a set of deliverables, and the needed hours/days to achieve established aims.

CMHI Consultation Services can be arranged and delivered via phone, web, on-site/face to face, e-mail, or as an individually designed and blended grouping of these services.
