The Medical Home TAPPP™ (Gap) Analysis is CMHI’s most up-to-date primary care development resource. The full TAPPP process:

• a) Includes a thorough, individualized and practice-based medical home assessment
• b) Provides a complete, customized scoring dashboard, and
• c) Delivers a report with practice recommendations and related care and educational tools.

…TAPPP™ is available for pediatrics, adult medicine, and as a merged resource for both pediatrics and adult care - for use across the lifespan. TAPPP™ is designed to assist each network or group with Medical Home transformation and, if appropriate, ensuing recognition.

Content expertise and advice - assisting with the clarification of medical home best practices; health care policy decision points; learning collaborative goals and methods; practice outreach and recruitment strategies; learning session content development and delivery; patient and family-centered engagement strategies; program implementation including care coordination and other medical home related subject matter.

Education related to all aspects of the Medical Home such as a history and background; awareness education; access and communication; development of team care; quality improvement; patient and family-centered communications and processes; metrics, outcomes, and evidence; approaches to special populations; the provision of planned and coordinated care; and examples of successful Medical Homes.

Coaching of coaches and/or facilitators helping them to promote medical home measurement, team development, practice improvement, implementation of change, staff and workforce skill building, measurement of progress, and more.
