Medical Home Family Index and Family Survey

CMHI believes that medical home activities cannot be fully measured and/or evaluated without including the family perspective. Therefore the Pediatric Medical Home Index and the Pediatric Medical Home Family Index and Survey are offered as a set, they are essential companions to one another. The Pediatric Medical Home Family Survey is a shortened version of the Pediatric Medical Home Family Index and Survey.

Guidelines

When using the MHI tools we request that you:

  1. Inform CMHI in writing (CMHI@crotchedmountain.org) of your intent.
  2. Consider sharing your practice and family data with us (confidential, de-identified) in order to benchmark. Please remove all practice and personal identifiers.

Tips for Measuring

  1. Collect data by surveying patients/families pre and post to evaluate your improvement efforts.
  2. Compare the patient/family survey results to your responses on the Medical Home Index.
    1. Do the patient/family responses match your responses?
    2. Are their opportunities for improvement?
  3. Consider sampling your most “high-risk” population.
  4. Compare results against Benchmark data.
