Medical Home Projects
Overview
Highlighted are several of CMHI's New Hampshire Medical Home Projects and some National Medical Home Projects. Partners of CMHI in many of these initiatives include the NH Endowment for Health, NH’s Title V program – Bureau of Special Medical Services (SMS), NH Citizen’s Health Initiative, Dartmouth Hitchcock Medical Center, NH Medical Society, NH Institute on Disability, NH Nurse Practitioner Association, NH Family Voices, other hospital or independently owned primary care practices, and families/consumers.
CMHI provides content expertise, education and consultation focused on medical home development to a variety of states, centers and initiatives including the American Academy of Pediatrics, the Commonwealth Fund, various Health Resources and Services Administration (HRSA) grantees, and numerous state/regional programs.
