| CMHIs primary care based
practice improvement model, its measurement tools and technical assistance
blend to form a consultation package supportive of organizations requesting
assistance in health system improvements around medical home concepts.
CMHI resources benefit state Medicaid and Title V Programs (children
with special health care needs) which are operating under federal
Healthy People 2010 agenda mandates to substantially improve services
for children with special health care needs. Other health care entities
including private managed care organizations will also benefit from
these quality improvement activities.
CMHI uses a quality improvement process with provider/parent teams
that examine the structures and processes of care to implement and
measure medical home improvements. Added value is found by including
parents of children with chronic conditions (consumers) on these
quality improvement teams. Tools to measure the performance of Medical
Homes include The Medical Home Index and The Medical Home Family
Index; data from the use of these tools assist teams in making practice
improvements.
The tool kit, Building a Medical Home: Improvement Strategies
in Primary Care for Children with Special Health Care Needs
is keyed to the Medical Home Index and lays out a methodology for
change with specific improvement strategies matched to the index
and detailed within the kit.

specific training or technical assistance. CMHI offers an organized
